The second part of the question is tricky. One of the earliest recorded rap battles took place in 1981. It was Kool Moe Dee vs. Busy Bee Starski at the Harlem World Club. I’m sure that if you put your mind to it, you can trace these rap beefs back much earlier than that. But the Dee vs. Starski feud set a big nostalgic precedent for competitive exchanges in hip-hop later down the line. Over the years, these competitive exchanges evolved from live rap battles to recorded diss tracks.
In the mid-90s, the East vs. West rivalry ended up in a terrible tragedy for Tupac Shakur and The Notorious B.I.G. Since then, the temperature in rap battles has come down a lot.
